Responsibilities
Provide comprehensive patient assessments, including vital signs, physical examinations, and medication administration using sterile techniques and medical terminology.
Administer treatments such as IV infusions, injections, catheter care, and tube feeding while ensuring adherence to HIPAA regulations and infection control protocols.
Document patient care accurately within electronic health record (EHR) systems like Epic, Cerner, or Athenahealth; maintain detailed medical records and medical documentation.
Assist with diagnostic evaluations including specimen collection/processing and basic medical imaging procedures when necessary.
Educate patients and their families on disease management, medication use, nutrition, and post-discharge care plans to promote health coaching and self-care.
Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of home & community care services for individuals with disabilities or complex needs such as dementia or Alzheimer's care.
Respond promptly to triage calls or urgent situations involving acute pain management or infectious disease concerns; utilize critical care experience and emergency medicine skills as needed.
Requirements
Valid licensure as a Licensed Practical Nurse (LPN) in the state of practice.
Proven experience in diverse healthcare settings such as hospital medicine, ICU, emergency room, nursing home, hospice care, or post-acute care environments.
Familiarity with EMR/EHR systems including Epic, Cerner, eClinicalWorks or Athenahealth for accurate documentation.
Knowledge of ICD-9/ICD-10 coding for medical records and billing purposes.
Strong understanding of physiology, anatomy knowledge, patient assessment techniques, and vital signs monitoring.
Experience working with individuals with developmental disabilities or behavioral health needs is preferred.
Ability to perform IV insertion/injection administration, phlebotomy, catheterization, suturing, and sterile processing procedures confidently.
Excellent communication skills for effective patient education and team collaboration; ability to work independently in home settings while maintaining safety standards such as HIPAA compliance. Pay: $45.00 - $55.00 per hour
