About This Position

Overview Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) specializing in Home Health Care! In this vital role, you will deliver compassionate, high-quality nursing services directly to patients in their homes, promoting health, independence, and well-being. Your expertise will support individuals across diverse settings, including long-term care facilities, assisted living communities, and acute care environments. Responsibilities

  • Provide comprehensive patient assessments, including vital signs, physical examinations, and medication administration using sterile techniques and medical terminology.
  • Administer treatments such as IV infusions, injections, catheter care, and tube feeding while ensuring adherence to HIPAA regulations and infection control protocols.
  • Document patient care accurately within electronic health record (EHR) systems like Epic, Cerner, or Athenahealth; maintain detailed medical records and medical documentation.
  • Assist with diagnostic evaluations including specimen collection/processing and basic medical imaging procedures when necessary.
  • Educate patients and their families on disease management, medication use, nutrition, and post-discharge care plans to promote health coaching and self-care.
  • Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of home & community care services for individuals with disabilities or complex needs such as dementia or Alzheimer's care.
  • Respond promptly to triage calls or urgent situations involving acute pain management or infectious disease concerns; utilize critical care experience and emergency medicine skills as needed.

    Requirements

  • Valid licensure as a Licensed Practical Nurse (LPN) in the state of practice.
  • Proven experience in diverse healthcare settings such as hospital medicine, ICU, emergency room, nursing home, hospice care, or post-acute care environments.
  • Familiarity with EMR/EHR systems including Epic, Cerner, eClinicalWorks or Athenahealth for accurate documentation.
  • Knowledge of ICD-9/ICD-10 coding for medical records and billing purposes.
  • Strong understanding of physiology, anatomy knowledge, patient assessment techniques, and vital signs monitoring.
  • Experience working with individuals with developmental disabilities or behavioral health needs is preferred.
  • Ability to perform IV insertion/injection administration, phlebotomy, catheterization, suturing, and sterile processing procedures confidently.
  • Excellent communication skills for effective patient education and team collaboration; ability to work independently in home settings while maintaining safety standards such as HIPAA compliance.
