About This Position
Job Description Summary
As an LVN, you will be responsible for providing quality nursing care to patients in the comfort of their own homes. This hybrid position includes a combination of field visits and in-office responsibilities, offering a balanced and dynamic work environment.
Our office hours are Monday through Friday, 8:30 AM to 5:00 PM. While field schedules may vary based on patient needs, this is a full-time role aligned with standard business hours.
Responsibilities
Provide skilled nursing care to patients in their homes according to physician orders and established care plans
Administer medications and treatments as prescribed by the physician
Monitor and record vital signs, symptoms, and changes in patient condition
Communicate regularly with physicians, family members, and other healthcare professionals regarding patient status and progress
Educate patients and families on proper healthcare techniques and disease management
Maintain accurate and complete patient records
Participate in in-office duties including documentation, care coordination, and team collaboration
Collaborate with the healthcare team to ensure all patient needs are met
Qualifications
Must be a Licensed Vocational Nurse (LVN) with a current and active California license
Experience in home health or a related healthcare setting preferred
Ability to work independently and manage a flexible schedule between field and office responsibilities
Strong communication and interpersonal skills
Excellent critical thinking and problem-solving abilities
Reliable transportation and a valid driver’s license
Pay: $30.00 - $35.00 per hour
