Overview

Join our dynamic healthcare team as a Home Health Registered Nurse or Licensed Practical Nurse, where your expertise will directly impact patient well-being in the comfort of their homes. Duties

  • Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories, to develop individualized care plans.
  • Perform specialized procedures such as wound suturing, catheterization, ostomy care, and ventilator management with precision and adherence to safety protocols.
  • Coordinate care with multidisciplinary teams , ensuring accurate and timely documentation.
  • Provide patient education on disease management, medication administration, nutrition, and lifestyle modifications to promote health literacy.
  • Manage complex cases including hospice & palliative medicine, behavioral health needs, trauma medicine, and infectious disease care.
  • Respond promptly to triage calls and urgent situations in the home setting, demonstrating critical care response procedures and emergency response skills.

    Experience

  • Valid RN or LPN licensure with current state registration required
  • Demonstrated experience working with diverse populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia care needs.
  • Prior home care or community-based nursing experience preferred; familiarity with long-term care settings like nursing homes or assisted living facilities is highly valued.
  • Proficiency experience with ICD coding (ICD-9/ICD-10) and medical documentation standards is advantageous.
  • Strong clinical background in areas such as critical care , emergency medicine, internal medicine, or hospital medicine enhances your capability to handle acute situations confidently.
  • Knowledge of infection control practices, laboratory procedures including phlebotomy and specimen collection, and advanced skills like tube feeding or trauma assessment are beneficial.
  • Excellent communication skills for patient education, case management coordination, and collaboration within multidisciplinary teams.

    Experience working with individuals requiring tube feeding, Collaborate with healthcare professionals, Nursing, End-of-life planning and palliative care assessment, Vital signs, Experience working with individuals with neurodevelopmental disorders, Hospital medicine, RN License, Hospice experience, Initial assessment (medical evaluation), Tube feeding, Disease management patient education, Experience working with individuals with cognitive disabilities, Infectious disease care, Working with pediatric populations, Mechanical ventilation monitoring, Working with geriatric patients, Patient stabilization in emergency medicine, Palliative and hospice care teams experience, Care documentation, Mid-level, Developmental disability care assistance, Diagnostic evaluation, Healthcare infection prevention and control expertise, Managing patients as a nurse case manager, Clinical laboratory experience, Emergency medical trauma care, ICD-9, ICD-10, Patient history taking
