About This Position
QualificationsRequired
Graduate of an accredited school of nursing.
Current, unrestricted Registered Nurse (RN) license in the state of practice.
Minimum of one year of recent clinical nursing experience (home health, acute care, medical-surgical, rehabilitation, or related experience preferred).
Current BLS/CPR certification.
Proficiency with Data Soft Logic (DSL) Electronic Medical Records (EMR) and the ability to accurately complete OASIS assessments, clinical documentation, scheduling, and care planning within the system.
Valid driver's license, reliable transportation, and proof of automobile insurance.
Ability to meet all agency health screening and background check requirements.
Preferred
Previous home health or community health nursing experience.
Knowledge of Medicare Conditions of Participation and home health regulations.
Experience with OASIS-E documentation and Medicare billing requirements.
Wound care certification, IV therapy, or infusion therapy experience.
Knowledge, Skills, and Abilities
Strong clinical assessment and critical thinking skills.
Working knowledge of Data Soft Logic (DSL) EMR for documentation, patient records, care planning, and regulatory compliance.
Knowledge of nursing standards of practice and evidence-based care.
Excellent communication and patient education skills.
Ability to prioritize workload and manage time effectively.
Ability to work independently while collaborating with an interdisciplinary team.
Strong organizational and documentation skills.

Pay: $100.00 - $150.00 per hour
