Position Overview
As a Weekend RN Case Manager, you will provide skilled nursing care and complete admissions for patients requiring home health services. You will collaborate with physicians, families, and interdisciplinary team members to ensure continuity of care and positive patient outcomes.
Responsibilities Include:
Complete weekend admissions and in-home assessments
Determine eligibility for home health services
Develop and implement individualized care plans
Coordinate care with physicians and other healthcare professionals
Perform skilled nursing services, including wound care and medication management
Educate patients and caregivers on diagnoses, treatments, and recovery goals
Monitor patient progress and communicate changes to the care team
Ensure compliance with agency policies and regulatory standards
Promote patient independence, safety, and quality of life
Qualifications
Required:
Current Registered Nurse (RN) license in Nebraska or compact state
Minimum of one year of nursing experience
Previous Home Health experience required
Current BLS/CPR certification
Valid driver's license, reliable transportation, and current auto insurance
Preferred:
Experience with OASIS documentation
Strong assessment, communication, and time-management skills
