A LVN / Licensed Vocational Nurse is a member of the home care team who works under the supervision of a registered nurse and performs various personal care services as necessary to meet the client's needs. Job Summary: We are seeking a dedicated Visiting Nurse to provide high-quality healthcare services to patients in their homes. The ideal candidate will have experience in various medical procedures and possess exceptional patient care skills.
Responsibilities: - Conduct physical examinations and assessments - Provide skilled nursing care, including tube feeding, ventilator care, and airway management - Administer medications and treatments as prescribed by physicians - Monitor and record vital signs of patients - Perform pediatric care as needed - Develop and implement individualized care plans - Educate patients and families on proper healthcare practices - Collaborate with healthcare professionals for comprehensive patient care - Ensure infection control measures are strictly followed - Assist in discharge planning for patients transitioning from hospital to home care
Skills: - Proficiency in conducting physical examinations and assessments - Experience with tube feeding, ventilator care, and airway management - Knowledge of pediatric care practices - Familiarity with ICD-9 coding system - Strong understanding of infection control protocols - Ability to perform vital sign monitoring accurately - Excellent communication and interpersonal skills for patient education and family support
