About This Position

Job Description & Requirements RN Case Manager StartDate: 8/17/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Coordination of care across the continuum. Conduct initial assessments of patients in OBS or IP LOC, develop viable discharge plans with patients/family members/care givers to ensure a safe discharge plan. Work with the multi-disciplinary team to coordinate care of patient; minimize unnecessary use of resources. Will work with utilization review nurses to monitor authorization of care and case management assistants for the post-acute referral process. Complete CM initial assessment to evaluate needs for discharge planning and coordinate care across the continuum. Participate in multidisciplinary rounds. Meet with patients, family members or other designated participants in care; assist with authorization of post-acute care; May complete utilization reviews when needed or assigned.

MINIMUM REQUIRED QUALIFICATIONS

  • Active and unincumbered CA RN license
  • AHA BLS
  • 2+ years case management experience at a level 1 trauma facility
  • 2+ years inpatient care experience-non-ED-non-clinic
  • 2+ years traveling experience

    PREFERRED QUALIFICATIONS –

  • BSN
  • Inpatient Acute Care within the last 5 Years

    LENGTH OF ASSIGNMENT – 13 weeks

    SHIFT / HOURS PER WEEK – 8:30a- 5:00p and every other weekend

    SYSTEMS – Epic

    START DATE – ASAP

    Inpatient experience, RN License, BLS Certification, Travel nursing, Managing patients as a nurse case manager, Acute care facility experience
