About This Position

Overview

Join our dynamic healthcare team as a Home Health Clinical Supervisor, where you will lead and coordinate comprehensive home health services to ensure exceptional patient care. This vital role combines clinical expertise with leadership, overseeing nursing staff, managing patient assessments, and ensuring compliance with healthcare regulations. Duties

Supervise and support nursing staff, including registered nurses and home health aides, ensuring adherence to clinical policies and procedures.

Oversee patient care planning, including assessments, medication management, IV infusion protocols, and wound care such as catheterization and tube feeding.

Coordinate interdisciplinary team activities to develop personalized care plans that meet patients’ medical and psychosocial needs.

Ensure compliance with all state healthcare regulations, HIPAA privacy standards, and accreditation requirements like NCQA standards.

Manage documentation using electronic health record (EHR) systems such as Epic or Cerner, maintaining accurate medical records and medical documentation.

Conduct regular staff evaluations, provide coaching on clinical best practices including infection control, sterile processing, and aseptic technique.

Facilitate discharge planning, utilization review, and case management to optimize patient outcomes across settings like assisted living facilities, nursing homes, or long-term care environments.

Skills

Extensive experience working with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or behavioral health needs.

Proficiency in EMR/EHR systems such as Epic or Athenahealth for documentation and care coordination.

Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques.

Demonstrated leadership skills in supervising teams within acute care settings like ICU or Level I/II trauma centers; hospital medicine experience is highly valued.

Expertise in critical care procedures including ventilator management, airway management, IV insertion/injection administration, catheter care, phlebotomy and specimen collection/processing.

Familiarity with state healthcare regulations governing home health services and long-term care regulations.

Excellent communication skills for managerial communication with staff members while maintaining patient-centered focus; ability to triage effectively during emergencies or urgent situations.
