Join Our Team as a Registered Nurse – Case Manager in Mission Viejo, CA

We are seeking a dedicated and experienced Registered Nurse to join our dynamic Case Management team. Shift & Schedule

  • Day Shift: Monday through Friday
  • Hours: 8:00 AM – 4:30 PM (5 days a week, 8 hours each)

Requirements

  • Minimum 2 years of acute medical care management experience performing care coordination
  • Experience working with Epic charting system is required
  • Travel experience is mandatory for this position (HARD STOP)
  • Preferred experience in Trauma Level I and II care settings
  • Acute Hospital experience in Emergency Department, ICU, and Surgical Services is required
  • Proficiency in case management and utilization review skills including:
    • Appeals and Denials
    • Care coordination
    • Disability case management
    • Discharge planning
    • Disease management
    • Needs assessment and ordering Durable Medical Equipment (DME)
    • Plan of Care development and implementation
    • Worker’s compensation case management
  • Strong understanding and adherence to regulatory requirements including:
    • CMS (Centers for Medicare & Medicaid Services)
    • Department of Health standards
    • Diagnosis Related Groups (DRG)
    • HIPAA guidelines
    • OSHA regulations
    • The Joint Commission standards including Core Measures and National Patient Safety Goals
    • Workers Compensation regulations
  • Additional skills: familiarity with extra checklist requirements and strong Epic system expertise

Key Responsibilities

  • Coordinate and manage acute medical case load leveraging your expertise to ensure optimal patient outcomes
  • Perform comprehensive needs assessments and develop individualized plans of care
  • Facilitate discharge planning, including ordering DME and coordinating follow-up services
  • Manage and review appeals and denials related to case management
  • Collaborate effectively with multidisciplinary teams across Emergency, ICU, and Surgical Services units
  • Maintain full compliance with regulatory standards and documentation requirements using Epic
  • Use your travel experience to provide case management services across multiple hospital settings as required
  • Support quality improvement initiatives to meet Joint Commission and CMS standards

Qualifications

  • Current and valid Registered Nurse (RN) license
  • Minimum of 2 years’ experience in acute medical care management with proven skills in care coordination
  • Demonstrated experience and proficiency in Epic electronic health record system
  • Strong knowledge of case management principles across acute hospital environments including emergency, ICU, and surgical units
  • Excellent communication, organizational, and problem-solving skills
  • Ability to work independently and collaboratively within a fast-paced clinical setting
  • Commitment to maintaining compliance with all regulatory and safety requirements
  • Willingness to travel as required to meet role demands

Compensation & Benefits

Hourly Rate: $59.00 - $70.00
