Join Our Team as a Registered Nurse – Case Management

We are seeking a skilled and compassionate Registered Nurse to join our Case Management team in Mission Hills, CA. Shift/Schedule

  • Day Shift: Monday to Friday (5 days per week)
  • Hours: 8:00 AM – 4:30 PM (8-hour shifts)

Requirements

  • Minimum of 2 years acute care hospital case management experience performing care coordination and discharge planning
  • Proficient in Epic charting system (required)
  • Travel experience required; first-time travelers will be considered
  • Experience working in an acute hospital setting
  • Strong knowledge and application of care coordination, discharge planning, plan of care development
  • Ability to utilize InterQual Criteria (required)
  • Familiarity with CMS (Centers for Medicare and Medicaid Services) regulations
  • Understanding of DRG (Diagnosis Related Groups) is required

Key Responsibilities

  • Perform comprehensive care coordination and case management activities for patients in an acute hospital setting
  • Develop and implement individualized discharge plans to ensure smooth transitions from hospital to home or other care facilities
  • Utilize InterQual Criteria to assess patient needs and ensure appropriate level of care services
  • Collaborate proactively with multidisciplinary healthcare teams including physicians, nursing staff, social workers, and external providers
  • Maintain meticulous and timely documentation within the Epic electronic health record system
  • Ensure compliance with CMS regulations and DRG documentation requirements
  • Provide patient and family education regarding the plan of care and available resources
  • Support quality improvement initiatives by identifying barriers to patient care and recommending solutions

Qualifications

  • Active Registered Nurse (RN) license in the state of California
  • Minimum 2 years of acute care hospital case management experience
  • Demonstrated expertise in care coordination, discharge planning, and utilization review
  • Experience working with Epic Electronic Health Record system
  • Strong interpersonal and communication skills with the ability to work collaboratively across interdisciplinary teams
  • Knowledge of CMS guidelines, DRG coding, and InterQual criteria application
  • Willingness and ability to travel as part of job responsibilities
