Registered Nurse

Registered Nurse (RN) / Licensed Practical Nurse (LPN) – Home Health & Community Outreach Join the Home Instead Team! Home Instead is seeking a compassionate and motivated RN or LPN to provide in-home patient care while also helping grow our nursing and home care services throughout our communities. This unique role combines direct patient care with relationship-building and community outreach. Responsibilities Clinical Care Perform in-home nursing visits and patient assessments Develop and update care plans Provide skilled nursing services as permitted by licensure Educate clients and families on disease management and health needs Coordinate care with physicians and other healthcare professionals Maintain accurate and timely documentation Community Outreach & Marketing Build and maintain relationships with physicians, hospitals, clinics, social workers, case managers, and discharge planners Conduct regular visits to healthcare facilities and referral sources Promote Home Instead's skilled nursing and non-medical home care services Participate in community events, networking opportunities, and educational presentations Assist with increasing referrals and growing the client census Qualifications Current South Dakota RN or LPN license in good standing Excellent communication and interpersonal skills Strong organizational and time management abilities Ability to work independently and as part of a team Valid driver's license and reliable transportation Experience in home health, case management, or community outreach preferred but not required What We Offer Flexible scheduling Mileage reimbursement Supportive team environment Opportunity to make a meaningful impact in the lives of seniors and their families Professional growth and development opportunities If you are passionate about caring for others and enjoy building relationships within the healthcare community, we would love to hear from you!
