Physician Assistant / Nurse Practitioner – Outpatient Clinic
Wagner IHS Healthcare Center in Wagner, South Dakota is seeking an experienced Physician Assistant or Nurse Practitioner for a full-time outpatient locum tenens assignment supporting Indian Health Service operations. This role provides primary and same-day care for a rural patient population in a busy outpatient setting and is ideal for providers with strong family medicine or general medicine experience.
The provider will deliver outpatient care for patients across the lifespan, perform assessments and diagnoses, prescribe and manage treatment plans, coordinate referrals, document in RPMS/EHR, and maintain compliance with IHS, CMS, HIPAA, and facility standards. The clinic operates seven days a week from 7:00 a.m. to 11:00 p.m., with shift and block scheduling options available.
Qualified candidates must be licensed and unrestricted as a Nurse Practitioner or Physician Assistant in at least one U.S. jurisdiction, nationally certified, BLS certified, and have at least 3 years of recent Family Medicine or General Medicine experience. Prior outpatient experience, comfort in a rural setting, and strong documentation skills are important for success in this role.
This assignment offers the opportunity to serve a tribal community with meaningful clinical impact while working in a collaborative IHS setting. Candidates must be able to meet credentialing, background, fingerprinting, health screening, and onboarding requirements before start.
Overview
Wagner Indian Health Center is an ambulatory clinic within the Great Plains Area Indian Health Service and provides care to approximately 23,221 tribal members in a rural South Dakota service area. The clinic operates daily from 7:00 a.m. to 11:00 p.m., including weekends and federal holidays, and offers services such as outpatient care, pharmacy, laboratory, radiology, dental, public health nursing, optometry, pain management, behavioral health, OB/GYN, and telemedicine specialties.

Responsibilities
· Provide on-site outpatient medical services for patients from newborn through geriatric populations in the Wagner IHS Outpatient Department.
· Examine, diagnose, treat, prescribe, educate patients, and document care in accordance with approved clinical privileges.
· Use RPMS/EHR to enter and sign medical orders, document visits, and complete records within required timeframes.
· Order and review appropriate tests, obtain relevant medical information, and record pertinent normal and abnormal findings in the patient medical record.
· Follow IHS Purchased/Referred Care policies for referrals and consultations with specialty practitioners.
· Follow the IHS formulary and obtain approval when non-formulary medications are needed.
· Maintain compliance with HIPAA, CMS standards, IHS policies, infection control standards, and facility requirements.
· Support quality improvement efforts and maintain professional communication with medical staff and leadership.
Schedule
Coverage is needed for the Outpatient Department Sunday through Saturday from 7:00 a.m. to 11:00 p.m. The solicitation notes that regular workweek, weekend, shift coverage, and block scheduling hours may be used.
Minimum Qualifications
· Current active and unrestricted NP or PA license in at least one U.S. state or territory.
· National certification in the applicable professional discipline.
· Graduation from an accredited Nurse Practitioner or Physician Assistant program.
· Minimum of 3 years of Family Medicine or General Medicine experience within the past 36 months before starting.
· Current Basic Life Support certification.
· Computer proficiency: electronic health record experience preferred.
Credentialing and Compliance
Providers must be eligible for clinical privileges at Wagner IHS and submit required credentialing materials promptly after award, including licensure, board certification, BLS, resume, DEA certificate, continuing education, physical exam documentation, and other privileging documents identified by the facility.
The contract also requires successful background checks, fingerprinting, health clearance, immunization documentation, and compliance with privacy, security, and Indian Child Protection requirements before starting work.
Pay: $112.00 per hour
