We are seeking a dedicated and skilled Therapy/Rehabilitation professional to join our dynamic team in the PT Acute Care unit. Shift & Schedule

  • Day Shift: Monday to Friday
  • Hours: 8:00 AM to 5:00 PM (5 days a week, 8-hour shifts)

Requirements & Skills

  • Minimum 1 year of relevant experience
  • Proficiency with Epic charting system
  • BLS Certification required
  • Experience working in the following settings is required:
    • Acute Care
    • Outpatient Clinic
    • Rehabilitation Hospital
    • Skilled Nursing Facility (SNF)
  • Assessment skills including Body Mechanics
  • Orthopedic expertise with required knowledge on:
    • Cervicalgia
    • Low back pain
    • Spinal Conditions
    • Gait training
    • Hip fractures/injuries
    Experience with post-operative care including cervical surgery, shoulder repair, spinal surgery, total hip replacement, and total knee replacement is advantageous.
  • Neurologic patient care knowledge covering:
    • Cognitive disorders
    • Multiple Sclerosis
    • Muscular dystrophy
    • Parkinson’s disease
    • Post Stroke Rehabilitation
    • Spinal cord injury
    • Traumatic Brain injury
  • Professional skills involving:
    • Home Exercise Program development
    • Patient and family education
  • Specialty patient populations:
    • Neurology
    • Orthopedics
  • Comprehensive documentation proficiency including:
    • Assessment
    • Discharge
    • Evaluation
  • Experience using key equipment and devices such as:
    • Bed alarms
    • Call lights
    • Gait belts
    • Hospital/Specialty beds
    • Sliding boards
  • Billing knowledge including commercial, Medicaid, and Medicare A
  • No travel experience required

Key Responsibilities

  • Deliver comprehensive therapy and rehabilitation services to patients within acute care and outpatient settings, ensuring adherence to individualized treatment plans.
  • Conduct thorough patient assessments focusing on body mechanics, neurologic, and orthopedic conditions to inform effective interventions.
  • Apply expert knowledge to treat a broad range of patient populations, including those recovering from orthopedic surgeries and managing neurologic disorders.
  • Develop and implement home exercise programs and provide patient and family education to promote ongoing recovery and wellness post-treatment.
  • Utilize clinical expertise and specialized equipment to support safe mobility and optimize patient outcomes.
  • Maintain accurate and timely documentation of assessments, evaluations, and discharge plans using the Epic charting system.
  • Collaborate with interdisciplinary teams to ensure the delivery of integrated care and support appropriate billing processes for commercial, Medicaid, and Medicare A services.
  • Comply with all regulatory standards and agency policies to provide safe and effective therapy services.

Qualifications

  • Valid license or certification to practice as a therapy/rehabilitation professional as applicable.
  • Current BLS (Basic Life Support) certification.
  • Proven experience in acute care, outpatient clinic, rehabilitation hospital, and SNF settings.
  • Strong clinical knowledge in orthopedics and neurology patient populations.
  • Excellent communication and patient education skills.
  • Detail-oriented with strong documentation and organizational capabilities.

Compensation & Benefits

  • Competitive hourly rate of $55.00 - $65.00
