Join Our Team as a Registered Nurse – Case Manager in Santa Rosa, CA

We are seeking a dedicated and experienced Registered Nurse to join our dynamic Case Management team. Schedule & Shift Information

  • Day Shift: Monday through Friday
  • Hours: 8:00 AM – 4:30 PM (5 x 8-hour shifts)

Requirements & Skills

  • Minimum 2 years acute hospital Case Manager experience performing care coordination and discharge planning in a very fast-paced setting
  • Patient ratio experience: 1:20
  • Proficient with Epic electronic charting system (required)
  • Travel experience required; candidates will be screened for relevant experience
  • Current BLS certification
  • Experience across multiple work settings including acute hospital, ambulatory, behavioral health, home health, hospice, insurance company, and long-term acute care/rehab/skilled nursing
  • Proficient in case management/utilization review functions including admission criteria, benefits eligibility, care coordination, data abstraction, determining medical necessity per evidence-based guidelines, disability case management, discharge planning, disease management, needs assessment and ordering durable medical equipment (DME), plan of care development, prior authorizations, InterQual criteria utilization, and worker’s compensation case management
  • Acute hospital experience in specialties such as behavioral health, emergency department, ICU, NICU, pediatrics, SDU/PCU/IMC/observation units
  • Knowledge of regulatory requirements including CMS, CPT coding and billing, Department of Health standards, DRG, HIPAA guidelines, ICD-10 coding, NCQA, OSHA, The Joint Commission core measures and national safety goals, and workers compensation regulations
  • Demonstrated ability to utilize checklists and follow evidence-based protocols
  • Preferred experience in teaching hospitals, community hospitals, LTAC, Trauma Level I and II settings

Key Responsibilities

  • Coordinate comprehensive care plans facilitating optimum patient outcomes through effective assessment, planning, implementation, and evaluation
  • Manage complex discharge planning and transitions of care to ensure safe and timely patient discharges
  • Perform thorough patient assessments and needs evaluations, including ordering durable medical equipment as appropriate
  • Utilize Epic system for accurate and timely documentation and data abstraction
  • Maintain compliance with all regulatory and accreditation standards related to case management documentation and processes
  • Collaborate closely with interdisciplinary teams including physicians, social workers, therapists, and external providers to facilitate coordinated care
  • Review and apply admission criteria, benefits eligibility, and medical necessity guidelines such as InterQual to support appropriate utilization review
  • Manage utilization review processes including prior authorizations and worker’s compensation case management
  • Serve as a resource and educator for patients and families regarding disease management and community resources
  • Adapt case management practices to various healthcare settings including ambulatory, behavioral health, home health, hospice, and long-term care environments

Qualifications

  • Current and active Registered Nurse license
  • BLS certification is mandatory
  • Minimum 2 years acute hospital case management experience at a patient ratio of 1:20
  • Proficiency with Epic charting system and electronic medical records
  • Experience in multiple care settings and clinical specialties as outlined in requirements
  • Strong knowledge of healthcare regulations, coding, billing, and quality assurance standards
  • Excellent communication, organizational, and interpersonal skills
  • Ability to handle multiple priorities in a fast-paced environment while maintaining attention to detail

Compensation & Additional Information

  • Hourly rate: $59.00 - $70.00
