Shift & Schedule

  • Day Shift: Monday to Friday
  • Hours: 8:00 AM – 4:30 PM (5 days, 8 hours each)

Requirements & Skills

  • Minimum 2 years of experience in Acute Care Case Management / Discharge Planning
  • Patient ratio experience of approximately 1:25
  • Proficiency with Epic charting system (required)
  • Travel experience required – must be a seasoned traveler
  • Experience working in acute hospital settings
  • Strong knowledge of case management and utilization review including:
    • Admission criteria
    • Benefits eligibility
    • Care coordination
    • Discharge planning
    • Disease management
    • Needs assessment and ordering durable medical equipment (DME)
    • Plan of care development
    • Prior authorizations
  • Familiarity with regulatory requirements, including:
    • CMS (Centers for Medicare and Medicaid Services)
    • Department of Health regulations
    • Diagnosis Related Groups (DRG)
    • The Joint Commission standards, Core Measures, and National Safety Goals
  • Acute hospital experience in specialties such as:
    • Emergency Department
    • Intensive Care Unit (ICU)
    • Medical-Surgical (MS)
    • Surgical Services
  • Ability to complete additional required checklists and adhere to detailed procedural guidelines

Key Responsibilities

  • Provide expert case management services to acute care patients, ensuring appropriate discharge planning and continuity of care
  • Coordinate with interdisciplinary teams to develop and implement individualized patient care plans
  • Review and apply admission criteria and benefits eligibility in coordination with utilization review
  • Manage prior authorizations and arrange necessary durable medical equipment and services
  • Ensure adherence to regulatory standards, including CMS, Department of Health, DRG guidelines, and Joint Commission requirements
  • Maintain accurate and timely documentation using the Epic electronic health record system
  • Serve as a dedicated point of contact for patients, families, and healthcare providers during the acute care episode
  • Engage in disease management and patient education to promote optimal health outcomes

Qualifications

  • Valid Registered Nurse (RN) license in good standing
  • Minimum two years of acute care case management or discharge planning experience
  • Proficient with Epic charting system and acute care documentation standards
  • Solid understanding of healthcare regulations and compliance requirements
  • Demonstrated ability to work effectively in fast-paced, patient-focused acute hospital settings
  • Strong communication, organizational, and critical thinking skills
  • Flexibility and readiness to travel as required
