Shift & Schedule
- Day Shift: Four 10-hour shifts per week
- Hours: 7:00 AM – 5:00 PM
Position Requirements
- Minimum of 1 year of experience with outpatient infusion
- Current Oregon Pharmacist License (required)
- Certification in Chemotherapy/Hazardous Drug handling (required)
- Proficiency with Pyxis medication dispensing system (required)
- Experience with home health infusion (required)
- Oncology experience (required)
- Preferred experience using Epic electronic charting system
- Preferred background in community hospital settings
- Ability to compound intravenous preparations accurately and safely
- Skilled in dosing high-alert medications and psychotherapeutic management
- Expertise in therapeutic medication monitoring and pharmacotherapy management of oncologic medical problems
Key Responsibilities
- Provide comprehensive pharmacotherapy management for oncology patients, ensuring safe and effective medication use
- Prepare and compound intravenous medications with strict adherence to safety protocols
- Dispense medications utilizing Pyxis systems accurately and efficiently
- Monitor and manage dosing of high-alert and psychotherapeutic medications, tailoring therapies to patient-specific needs
- Collaborate with interdisciplinary teams in ambulatory care, outpatient clinics, and inpatient settings to optimize patient outcomes
- Maintain thorough and accurate electronic medical records, preferably using Epic
- Ensure compliance with all regulatory and facility standards related to chemotherapy and hazardous drug handling
- Educate patients and healthcare staff on medication usage, side effects, and safety considerations
Qualifications
- Licensed Pharmacist in the state of Oregon
- Certification in Chemotherapy/Hazardous Drug management
- At least 1 year of outpatient infusion pharmacy experience
- Strong clinical knowledge in oncology pharmacotherapy
- Experience with compounding intravenous medications and managing high-alert drugs
- Familiarity with Pyxis dispensing systems and Epic charting preferred
- Excellent communication and collaboration skills within multidisciplinary teams
- Detail-oriented with a commitment to patient safety and quality care
