Position Summary
Black Hills Center for Aging is seeking a compassionate, organized, and experienced Licensed Practical Nurse (LPN) to serve as a Case Manager within our Adult Day Program. This position plays a vital role in ensuring the health, safety, and well-being of program guests through comprehensive care planning, clinical oversight, documentation, and staff supervision.
The LPN/Case Manager collaborates with guests, families, healthcare providers, and program staff to deliver person-centered care that promotes independence, dignity, and quality of life.
Essential Duties and Responsibilities
Care Planning & Case Management
Conduct assessments and participate in the development of individualized guest care plans.
Write, update, and maintain comprehensive care plans based on guest needs, goals, and health status.
Monitor guest progress and revise care plans as needed.
Coordinate services and communicate with families, healthcare providers, and community resources.
Ensure compliance with state regulations, program policies, and best practices in adult day services.
Documentation & Reporting
Maintain accurate and timely clinical records and documentation.
Complete required charting, incident reports, assessments, and progress notes.
Prepare reports related to guest care, attendance, and program outcomes.
Ensure confidentiality and security of all guest records.
Program Operations
Track attendance records, follow up on guest absences to ensure guest well-being, make efforts to maximize attendance, and communicate concerns regarding attendance patterns.
Assist with admission, enrollment, and discharge processes.
Staff Supervision & Leadership
Provide direct supervision and guidance to caregiving staff.
Monitor staff performance and adherence to care plans and program policies.
Assist with staff training, mentoring, and ongoing education.
Foster a positive, team-oriented work environment focused on quality care and guest satisfaction.
Assist with personal cares, showers, and transportation as needed.
Quality Assurance
Promote and maintain high standards of guest care and safety.
Identify opportunities for quality improvement and implement corrective actions as needed.
Participate in program evaluations, audits, and regulatory reviews.
Ensure guests receive compassionate, respectful, and person-centered services.
Qualifications
Current Licensed Practical Nurse (LPN) license in the State of South Dakota.
Experience in geriatrics, adult day services, long-term care, case management, or related healthcare settings preferred.
Strong assessment, documentation, and organizational skills.
Excellent communication and interpersonal abilities.
Ability to supervise and motivate staff while maintaining a collaborative team environment.
Proficiency with electronic health records and Microsoft Office applications.
Valid driver's license and reliable transportation preferred.
Knowledge, Skills, and Abilities
Understanding of aging-related health conditions and chronic disease management.
Knowledge of care planning principles and person-centered care practices.
Ability to manage multiple priorities and maintain accurate records.
Strong problem-solving and critical-thinking skills.
