Join Our Team as a Registered Nurse – Case Manager in Torrance, CA

We are seeking a dedicated and experienced Registered Nurse to join our Case Management team. This is an exciting opportunity for a healthcare professional passionate about providing quality care coordination and utilization review, supporting patients through their healthcare journeys across various acute and post-acute settings. Shift & Schedule

  • Day Shift: 5 days per week, 8 hours each (8:00 AM – 4:30 PM)

Requirements

  • Minimum of 2 years of acute care case management or discharge planning experience
  • Proficient with Epic charting system (required)
  • Demonstrated travel experience; open to screening first-time travelers
  • Experience in a variety of work settings, including:
    • Acute Hospital
    • Behavioral Health (required)
    • Home Health (required)
    • Hospice (required)
    • Insurance Company (required)
    • Long Term Acute Care, Rehab, Skilled Nursing (required)
  • Comprehensive knowledge of case management/utilization review practices including:
    • Admission Criteria (required)
    • Appeals and Denials (required)
    • Benefits Eligibility (required)
    • Care Coordination
    • Determining Medical Necessity per evidence-based guidelines (required)
    • Discharge Planning
    • Disease Management
    • Needs Assessment and Ordering Durable Medical Equipment (DME)
    • Plan of Care (required)
    • Prior Authorizations (required)
    • Utilization of InterQual Criteria (required)
    • Workers’ Compensation Case Management (required)
  • Utilization review expertise including:
    • Application of Admission Criteria (required)
    • Understanding Medical Necessity

  • Acute hospital experience in multiple specialties such as:
    • Behavioral Health (required)
    • Emergency Department (required)
    • Intensive Care Unit (ICU) (required)
    • Multiple Sclerosis (MS) (required)
    • Neonatal Intensive Care Unit (NICU) (required)
    • Pediatrics (required)
    • Pediatric Intensive Care Unit (PICU) (required)
    • Step-down Unit/Progressive Care Unit/Intermediate Care/Observation (SDU/PCU/IMC/Obs) (required)
    • Surgical Services (required)
    • Women’s Services (required)
  • Regulatory knowledge and compliance including CMS, DRG, HIPAA guidelines, and Workers’ Compensation (required)

Key Responsibilities

  • Perform comprehensive case management and utilization review services across acute and post-acute care settings ensuring appropriate patient care and resource utilization
  • Coordinate care plans collaboratively with interdisciplinary teams to optimize patient outcomes and facilitate timely discharge planning
  • Utilize Epic and other clinical tools effectively for accurate and timely documentation
  • Evaluate medical necessity and apply evidence-based guidelines including InterQual criteria in clinical decision-making
  • Manage appeals, denials, prior authorizations, and benefits eligibility to support patient access and compliance
  • Provide expert case management services in behavioral health, home health, hospice, insurance, long-term acute care, and rehab settings
  • Ensure compliance with all regulatory standards including CMS, HIPAA, and workers’ compensation requirements
  • Support the education and advocacy for patients and their families throughout the care continuum
  • Travel as required, demonstrating flexibility and adaptability to meet the demands of varied clinical settings

Qualifications

  • Valid Registered Nurse (RN) licensure in California
  • At least 2 years experience in acute care case management or discharge planning
  • Proficient in Epic electronic medical records system
  • Strong clinical assessment and critical thinking skills
  • Demonstrated knowledge of utilization review processes and regulatory requirements
  • Excellent communication, organizational, and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Willingness and ability to travel as needed
