About This Position

Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team. In this vital role, you will provide high-quality nursing care to residents in a skilled nursing or assisted living environment, ensuring their comfort, safety, and well-being. Your expertise will support the health needs of our residents, including those with complex medical conditions such as dementia, Alzheimer's, and other chronic illnesses. This position offers an exciting opportunity to make a meaningful difference in the lives of seniors and individuals with disabilities through attentive, personalized care.

Responsibilities

Administer medications and treatments accurately following physician orders, including injections, IV infusions, and catheter care.

Conduct comprehensive patient assessments by monitoring vital signs, evaluating symptoms, and documenting medical histories using Electronic Health Record (EHR) systems.

Assist with activities of daily living such as mobility support, tube feeding, wound care, and physical examinations to promote resident independence and comfort.

Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident’s needs.

Ensure compliance with long-term care regulations, HIPAA confidentiality policies, and infection control protocols while maintaining meticulous medical documentation.

Provide emergency response support including first aid, airway management, acute pain management, and triage for urgent health issues.

Communicate effectively with residents’ families and caregivers to update them on health status and coordinate ongoing care strategies.

Skills

Proven experience working with geriatric patients in settings such as skilled nursing facilities, assisted living communities, or long-term care environments.

Strong knowledge of medical terminology, physiology, ICD coding (International Classification of Diseases), and medication administration procedures.

Familiarity with EMR/EHR systems like Epic or Cerner for accurate documentation and patient record management.

Skilled in performing clinical procedures including venipuncture, catheterization, sterile processing techniques, and specimen collection/processing.

Ability to handle complex cases involving dementia care, Alzheimer's disease management, hospice care considerations, and behavioral health challenges.

Experience working with individuals with developmental disabilities or special needs populations is highly desirable.

Excellent communication skills for caregiver interaction, patient assessment, case management, and family education.

Duration: 13 weeks

Shift: Nights 12*3

Schedule: 6:00 PM - 6:30 AM
