Responsibilities
- Provide skilled nursing care in a hospice setting under the guidance of the clinical team.
- Support patient comfort and quality of life through routine nursing assessments and observation.
- Administer medications and treatments as ordered and within scope of practice.
- Monitor vital signs and report changes in condition to the appropriate members of the care team.
- Assist with care coordination needs and maintain accurate documentation of care provided.
- Maintain a compassionate approach while supporting patients and families during end-of-life care.
Requirements
- Active LPN/LVN license
- Prior hospice experience (preferred)
- Ability to provide patient-centered care in a fast-paced clinical environment
- Strong communication and documentation skills
