Responsibilities
- Provide direct patient care within the scope of an LPN/LVN in a long-term care setting.
- Monitor patients’ condition and report changes to the appropriate clinical team.
- Administer medications and follow established medication safety practices.
- Assist with activities of daily living (ADLs) and support care plan goals.
- Maintain accurate, timely documentation of care provided.
- Support infection control and follow safety protocols to protect patients and staff.
- Communicate effectively with nurses and interdisciplinary team members to coordinate care.
