About This Position

Job Summary

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and comprehensive assessments. Duties

Perform thorough patient assessments, including vital signs, physical examinations, and health histories to determine care needs.

Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.

Monitor patients’ conditions closely, utilizing critical care experience when managing acute pain or unstable health situations.

Assist with diagnostic evaluations such as specimen collection, processing, and documentation using EMR/EHR systems like Epic or Cerner.

Provide specialized care for patients with dementia or Alzheimer’s disease in memory or hospice care settings.

Support airway management procedures including ventilator care and tube feeding for patients with complex needs.

Collaborate with interdisciplinary teams on case management plans for geriatrics, pediatrics, or individuals with developmental disabilities.

Maintain strict compliance with HIPAA regulations and clinical confidentiality policies while documenting medical records accurately.

Educate patients and their families on health coaching topics such as medication management, nutrition, and post-discharge care instructions.

Respond promptly to triage calls or urgent situations in emergency room or urgent care environments using first aid and basic life support skills.

Experience

Valid LVN license with current state certification required; additional certifications in CPR/BLS strongly preferred.

Proven experience working with diverse populations including seniors, pediatric patients, or individuals with disabilities.

Hands-on experience in various healthcare settings such as hospitals (including ICU or Level I Trauma Centers), skilled nursing facilities, hospice programs, or home & community care.

Demonstrated proficiency with electronic medical records (EMR/EHR) systems like Epic, Cerner, or eClinicalWorks for accurate documentation and data management.

Familiarity with advanced nursing procedures including IV infusion therapy, catheterization, sterile technique, and airway management.

Knowledge of ICD coding (ICD-9/10), CPT coding for procedures, and long-term care regulations to ensure compliance and proper billing practices.

Experience providing specialized dementia or Alzheimer’s care along with managing acute pain cases in a variety of clinical environments such as emergency rooms or surgical units.

Ability to work effectively within multidisciplinary teams providing patient-centered care across medical-surgical units or outpatient clinics.
