About This Position

We are looking for a strong LPN or RN with documented leadership experience in the assisted living/memory care environment to serve as our Community Support Nurse, primarily within our communities across the West Coast.

This position will facilitate the operation of the Residential Care Department to comply with state regulations and our Senior Living Policy and Procedures.

Do you have a serious background as the head of an assisted living department/memory care community and are open to traveling 80 % of your work week, where you will get to train and develop community-based nurses? You would oversee the delivery of health care and supportive services provided by the Residential Services Director, Memory Care Director, and the direct care staff by ensuring clinical excellence and survey readiness. Strong knowledge of state regulations is required.

Job Duties:

Assist and train the Residential Services Director and Memory Care Director in attaining the skills needed to meet the residents' care needs by establishing a person-centered approach to the planning and delivery of services and supporting residents’ rights to participate in decision-making.

Provide training to the Residential Services Director and Memory Care Director on the company-based tools for assessing each resident's health, functional, and psychosocial status.

Communicate and train staff on corporate protocols and participate in admission and retention of resident discussions. Be a creative problem solver in the best interest of our residents.

Periodic audit of clinical records or no less than twice annually for each community. Assist the community with plans of correction as needed.

Maintain up-to-date knowledge of all rules and regulations about resident care in the assigned state. Stay informed of industry and health trends and standards and apply new knowledge.

Treat all co-workers with respect and dignity at all times. Be a positive team member. Support the mission, vision, and culture of the organization through positive communication and leadership

Contribute to community relationships by demonstrating cooperation and professional conduct with residents, families, fellow associates, physicians, pharmacies, and other vendors or persons.

Interview residents and families before admission and coordinate pre-admission screening and assessment.

Evaluate resident care needs and determine the required level of care upon admission and changes in condition.

Accountabilities:

Maintain clear records of all resident changes, requests, and irregularities

Update medical records as needed and chart in the resident record as needed for

change of condition.

Observe the resident’s health status and act to address concerns within the scope of practice.

Record and notify the Executive Director, physician, pharmacist, and family of significant changes to the resident’s health status and/or reactions to drugs, treatments, or significant incidents.

Order medications from the pharmacy, maintain records, and ensure that medications are properly stored and cared for.

Review and implement incident reports; evaluate the need for intervention to minimize the risk of further occurrences

Perform other related assignments as required.

Report any suspected or observed abuse, neglect, or misappropriation of property or funds of a resident to the appropriate licensing agencies.

Be willing and able to work in any resident care position.

Education & Experience you need to qualify:

Must have a current unencumbered LPN or RN license.

Excellent written and oral community and motivational skills.

Well-organized and flexible.

A compact nursing license is preferred.

LPN or RN with consulting experience in Assisted Living or memory care with a minimum of 3 years in geriatric nursing, including medication delivery software.
