We are seeking a compassionate, clinically strong, and dependable Nurse Practitioner or Physician Assistant to serve patients throughout Palm Desert and Palm Springs.
This role is ideal for a provider who enjoys building long-term relationships with patients, collaborating with care teams, and delivering primary care in a mobile healthcare setting.
Position Overview
The Nurse Practitioner or Physician Assistant will provide direct primary care services to patients in senior living communities, including assisted living, memory care, and other residential care settings.
The provider will assess, diagnose, treat, and manage acute and chronic conditions while working closely with supervising physicians, facility staff, families, and internal care teams.
The ideal candidate is comfortable working independently in the field, demonstrates strong clinical judgment, and is committed to delivering reliable, patient-centered care to older adults.
Key Responsibilities
Provide primary care services to residents in senior living communities
Conduct patient assessments, physical exams, and routine follow-up visits
Diagnose and manage acute and chronic medical conditions
Order and interpret labs, imaging, and diagnostic tests as appropriate
Prescribe medications and develop treatment plans within scope of practice
Monitor changes in patient condition and coordinate appropriate interventions
Support preventive care, chronic disease management, and overall patient wellness
Collaborate with supervising physicians, facility staff, families, pharmacies, and interdisciplinary care teams
Communicate clearly and professionally with residents, families, and senior living community staff
Maintain accurate, timely, and compliant documentation
Support continuity of care through consistent follow-up and care coordination
Represent Optima Health with professionalism, compassion, and reliability
Follow all applicable clinical, regulatory, state, federal, and HIPAA requirements
Qualifications
Active California Nurse Practitioner or Physician Assistant license required
Experience in geriatrics, internal medicine, family medicine, or primary care preferred
One to two years of clinical experience preferred
Strong candidates with relevant training will also be considered
Experience working with older adults, assisted living communities, memory care, home health, or mobile care preferred
Strong clinical judgment and the ability to work independently
Excellent communication and interpersonal skills
Compassionate, patient-centered approach to care
Highly organized, dependable, and responsive
Comfortable working in a field-based mobile healthcare environment
Ability to collaborate effectively with physicians, care teams, facility staff, patients, and families
Reliable transportation required
Ability to travel regularly throughout Palm Desert and Palm Springs
