Licensed Vocational Nurse (LVN) – AM Shift

Schedule: Friday, Saturday & Sunday
Shift: AM Shift – 12-Hour Shifts
Position: Part-Time LVN
Serenity Congregate Care is seeking an experienced, dependable, and clinically strong Licensed Vocational Nurse (LVN) to join our nursing team. We are a small, high-acuity Congregate Living Health Facility (CLHF) providing individualized skilled nursing care in a home-like environment.
This position is ideal for an LVN who is confident working with higher-acuity patients, has strong clinical judgment, and can work independently while remaining an effective part of the interdisciplinary team.
Responsibilities
Provide direct skilled nursing care within LVN scope of practice
Administer medications and treatments accurately and timely
Complete thorough nursing assessments/observations and documentation
Monitor residents for changes in condition and promptly report concerns
Follow physician orders and established plans of care
Provide care for medically complex and higher-acuity residents
Maintain accurate and timely electronic medical records
Communicate effectively with physicians, RNs, families, pharmacies, and other members of the care team
Respond appropriately to changes in condition and urgent clinical situations
Follow infection prevention, medication management, safety, and facility policies
Maintain professional and compassionate interactions with residents and families
Complete assigned shift responsibilities before leaving the facility
Qualifications
Current California LVN license – required
Current BLS/CPR certification
Prior skilled nursing, subacute, CLHF, home health, or high-acuity experience strongly preferred
Strong medication administration and clinical skills
Experience with medically complex patients preferred
PCC/electronic charting experience preferred
Excellent documentation and communication skills
Ability to recognize and appropriately respond to changes in condition
Comfortable working in a small facility where teamwork and accountability are essential

Schedule availability must include Friday, Saturday, and Sunday AM shifts (12 hours).
