Job description:

Hemet Valley Recovery Center is currently seeking a per diem LVN!

HVRC is a full continuum of care; detoxification, residential treatment and partial hospitalization program. HVRC detox unit is a 21-bed general acute care unit specializing in addiction medicine. The interdisciplinary team assists patients safely through a medical detox, while also addressing medical comorbidities associated with substance abuse. Our emphasis is placed on preparing and transitioning patients to the next level of care for their continued recovery process.

Qualifications:

· Possession of current LVN license

· Minimum of 2 years of clinical experience in substance abuse, acute care, and/or psychiatric setting preferred

· Admission, triage, and utilization review and/or case management experience preferred

· BLS certification required prior to start

Essential Duties and Responsibilities:

· The nurse demonstrates the ability to assess, plan, implement and evaluate patient care.

· Provides nursing care to patients in a chemical dependency setting within the LVN scope of practice; prepares patients and cares for them during treatment; administers medications and controls narcotics per hospital policy.

· Reviews assessment findings with physician and obtains admission orders

· Provides education and support to prospective patients and family members; discusses treatment options and provides referrals.

· Administer prescribed medications safely and on time; conducts CIWAs, COWS, and other withdrawal assessment tools to monitor and evaluate patient withdrawal symptoms.

· Completes accurate and timely electronic medical record (EMR) documentation.

·Collaborate with pharmacists, RNs, LVNs, CNAs, providers and Case managers.

· Gathers required demographic and insurance information as the first step in the admission process

· Utilizes clinical experience to complete comprehensive nursing assessments, including medical, substance abuse, and psychiatric histories

· Consults with physicians, clinical managers, and treatment team in making admission and/or referral decisions

· Reviews assessment findings with physician and obtains admission orders

· Provides a detailed overview of the detox process and services with clarity and expertise to prospective patients

· Provides education and support to prospective patients and family members. Discusses treatment options and provides referrals as needed

· Maintains, develops, and enhances referent relationships

· Completes insurance authorization and reviews

· Utilizes clinical expertise to conduct telephone screening and in-person nursing assessments

· Provides thorough documentation during screening process, assessment, and all other patient/family interactions
