As a Home Care Nurse (LPN/RN), you’ll be a vital member of our interdisciplinary team, delivering skilled, person-centered care both in participants’ homes and in our day center. If you value meaningful relationships, autonomy in your day, and truly making an impact, this role is for you.
What You’ll Do
Provide direct nursing care in home and day center settings
Assess participant needs and collaborate with providers and team members
Respond promptly to changes in condition or sudden illness
Maintain accurate, timely EMR documentation
Educate participants and caregivers on medications, chronic disease management, and wellness
Participate in an on-call rotation as needed
Adapt quickly and professionally to evolving care needs
Foster a supportive, positive environment for every participant

What You’ll Need
Associate or Bachelor’s degree in Nursing (ASN or BSN)
Current North Dakota nursing license (LPN or RN)
Current CPR/BLS certification
Valid driver’s license, good driving record, and required auto insurance
Strong communication, organization, and problem-solving skills
Ability to travel frequently for home visits in varying weather conditions
