About This Position

Job Summary

90-day $3000 SIGN-ON BOUNS!!!! We are seeking a dedicated and compassionate Resident Care Manager to lead and coordinate resident care services within our healthcare facility. In this vital role, you will oversee the delivery of high-quality patient care, ensuring that residents' needs are met with professionalism, empathy, and efficiency. Your leadership will foster a supportive environment for both residents and staff, promoting optimal health outcomes and enhancing the overall quality of life for those in your care. Responsibilities

  • Supervise and coordinate daily resident care activities, ensuring adherence to care plans and facility policies
  • Lead and support nursing staff, including scheduling, training, and performance evaluations
  • Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations
  • Oversee discharge planning, medical records management, and documentation in compliance with HIPAA regulations
  • Collaborate with interdisciplinary teams to develop individualized care strategies for residents with complex needs such as critical care, infectious disease management, or behavioral health issues
  • Monitor patient progress through diagnostic evaluations and adjust care plans accordingly
  • Facilitate intake processes for new residents and manage ongoing case management efforts to optimize post-acute and outpatient care pathways

    Requirements

  • Proven experience in critical care environments such as ICU, Emergency Room, or Level I/II Trauma Centers
  • Strong background in hospital medicine, medical-surgical nursing, or post-acute care settings
  • Knowledge of hospital physiology, medical terminology, and patient assessment techniques
  • Experience with managed care protocols, utilization management, discharge planning, and case management practices
  • Familiarity with pediatric (PICU), geriatrics, behavioral health, and outpatient clinic environments is preferred
  • Excellent communication skills with the ability to lead teams effectively and coordinate multidisciplinary efforts
  • Ability to handle sensitive patient information in compliance with HIPAA standards
  • Demonstrated ability to perform diagnostic evaluations and manage acute care situations effectively
