About This Position

Fastaff is hiring experienced Registered Nurse (RN) Case Managers for high-paying travel assignments across the United States. Earn up to $2,826 per week while making a meaningful impact on patient outcomes through care coordination, discharge planning, utilization management, and patient advocacy.

As a Travel RN Case Manager, you will develop, implement, and evaluate individualized patient care plans while coordinating services across the healthcare continuum. Job Responsibilities

Develop, implement, and monitor individualized patient care plans

Coordinate care between patients, families, healthcare providers, specialists, and community resources

Conduct clinical assessments and evaluate patient progress

Facilitate discharge planning and transitions of care

Advocate for patients to ensure timely access to appropriate healthcare services

Collaborate with physicians, nurses, social workers, and interdisciplinary care teams

Support utilization review, resource management, and quality improvement initiatives

Educate patients and families regarding treatment plans, healthcare resources, and disease management

Promote continuity of care and positive patient outcomes across healthcare settings

Maintain accurate documentation and compliance with facility and regulatory requirements

Minimum Qualifications

Active Registered Nurse (RN) license

Current BLS certification

Current ACLS certification

Minimum of 2 years of recent Case Management RN experience

Strong communication, critical thinking, and care coordination skills

Ability to work effectively in fast-paced healthcare environments
