About This Position

Resident Care Manager (RCM) - RN or LPN

Job Summary:
Washington Care Center in Seattle, WA is seeking a Resident Care Manager (RCM) to provide direct nursing care to residents and supervise the day-to-day activities performed by nursing assistants. Develops a plan of care for residents by evaluating and assessing the resident's complete needs. Will arrange and coordinate all services to insure residents plan is being met while reevaluating and making changes as necessary to insure resident health, safety and general wellbeing.

Essential Functions:

Supervises nursing care to residents.

Complies with state and Federal Regulations.

Participate in the development of a written plan of care for each resident and review resident’s medical and nursing treatments to ensure they are in accordance with the resident’s care plan and wishes.

Complete the planning, scheduling and revising of the MDS, including the implementation of RAPs and Triggers.

Make daily rounds to ensure that all nursing personnel are performing their work assignments.

Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided and the resident’s response to care and wishes.

Participate in reviews of the discharge plans and prepare reports for the Care Plan Committee as directed.

Delegate, train, evaluate and support RN, LPN and CNA personnel.

Provide direct nursing care as necessary.

Assist the Director of Nursing and fill in as needed.

Make recommendations for new or changes in procedures, policies, methods, education, reference material and general nursing practices to ensure the highest level of quality patient care is given.

Education, Licensure, & Experience Requirements:

Required: Graduate of an accredited school of nursing.

Required: Current WA or Compact RN/LPN licensure in good standing.

Required: 2+ years in supervisory/management position, including experience with budgets, staff development, training and scheduling

Required: Minimum 1 year of experience as a supervisor in a healthcare setting.

Required: Training in rehabilitative and restorative nursing practices.

Knowledge, Skills & Abilities:

Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities

Working knowledge of MDS process, state and federal survey, and CMS

Experience with Electronic Medical Records and computer documentation systems.
