Hospice Aide
Reports To: Director of Patient Care Services
Supervision: Registered Nurse Case Manager
Location: Field-based, Hybrid.
Position Summary
The Hospice Aide provides compassionate, hands-on personal care to hospice patients in their place of residence, following the individualized plan of care under the supervision of an RN Case Manager. This role supports patient comfort, safety, dignity, and quality of life while working closely with patients, families, and the interdisciplinary hospice team at Peaceful Hearts Hospice, LLC.

Qualifications
High school diploma or GED required; must be at least 18 years of age
Current, valid CNA/Aide certification or license required
Ability to communicate effectively in English, both verbally and in writing
Previous experience in hospice, home health, long-term care, or a related healthcare setting preferred
Compassionate, dependable, and professional approach to patient care
Strong communication and interpersonal skills
Ability to work independently while following care plans, company policies, and supervisor direction
Commitment to providing respectful, compassionate care to patients and their families
Key Responsibilities
Assist with bathing, grooming, dressing, toileting, transfers, ambulation, feeding, and light housekeeping
Take and report vital signs as directed
Observe and report changes in patient condition to the RNCM
Educate families on daily care between visits
Maintain timely documentation and communicate with the care team
Work Environment & Requirements
Field-based role in homes, assisted living, and skilled nursing facilities
Manage an average caseload of 10–15 patients
Valid driver’s license, reliable transportation, and auto insurance required
Ability to meet physical demands of patient care
