RN Care Manager - Long-Term Care
Location: Central SDA (Field-Based)

About the Role
We are seeking a compassionate and experienced RN Care Manager to support members with complex medical and long-term care needs. In this field-based role, you will coordinate care, develop personalized service plans, and work closely with members, caregivers, providers, and community resources to improve health outcomes while ensuring high-quality, person-centered care.
What You'll Do
Assess the needs of high-risk and medically complex members to develop individualized care plans
Coordinate care across providers, specialists, caregivers, and community organizations
Monitor member progress, identify changes in condition, and update care plans as needed
Advocate for members by ensuring access to appropriate healthcare services and benefits
Educate members and caregivers on disease management, treatment plans, available resources, and healthcare benefits
Review referrals, assessments, and clinical documentation to support care coordination
Identify trends and opportunities to improve quality, outcomes, and regulatory compliance
Coordinate service authorizations for long-term care services and community resources
Conduct home and community visits to assess member needs and provide ongoing support
Maintain accurate documentation in compliance with state, federal, and organizational guidelines
Collaborate with interdisciplinary teams to improve care delivery and member outcomes
Mentor and support new clinical staff during onboarding and training as needed
Required Qualifications
Active Registered Nurse (RN) license in the applicable state or Compact State License, or active Nurse Practitioner (NP) license
Graduate of an accredited School of Nursing or Bachelor's degree with 4–6 years of related healthcare experience
Experience in care management, case management, service coordination, or long-term care
Knowledge of complex medical conditions, care planning, and healthcare coordination
Strong communication, organization, and problem-solving skills
Ability to work independently in a field-based environment while managing multiple priorities
Proficiency with clinical documentation and electronic health record systems
Preferred Qualifications
Bachelor's degree in Nursing (BSN)
Experience within Managed Care, Medicaid, Medicare, or Long-Term Services and Supports (LTSS)
Experience working with high-acuity or medically complex populations
Resource Utilization Group (RUG) certification (or ability to obtain within required timeframe if applicable)
Experience mentoring or precepting clinical staff
