About This Position
Inland Valley Home Health Agency is currently looking for experienced, friendly, and reliable Licensed Vocational Nurses (LVN) who are interested in doing home health nursing visits. Nurses will be paid per visit. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong understanding of various medical conditions and the ability to work independently while collaborating with a multidisciplinary team.
Job Requirements:
Current California Board of Vocational Nursing license in good standing
At least one year of home health experience preferred and/or at least one year acute/subacute, or hospital experience
Current resume
Current CPR Card, TB, annual physical
Driver's License or State ID
Bilingual a plus + but not a requirement
Ability to use computerized charting
Proof of authorization to work, if applicable
Responsibilities (LVN):
Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
Administer medications and treatments as prescribed by physicians.
Provide education and health coaching to patients and their families regarding disease management, medication adherence, and lifestyle modifications.
Monitor patient progress and adjust care plans as necessary.
Maintain accurate patient records and documentation in compliance with healthcare regulations.
Collaborate with healthcare professionals to coordinate care and ensure optimal patient outcomes.
Complete progress notes in a timely manner
