About This Position
Job Summary
The LPN provides routine and emergency assessments and nursing care to ensure optimal health status for each individual we support. This role provides a casual environment with great PTO!
Shifts: Mon-Fri 8-5 pm, paid holidays off
Essential Duties & Responsibilities
Coordinate physician and health care services for people served
Conduct health care assessments and write health care plans
Facility training and orientation of new staff regarding healthcare procedures
Provide assistance to staff as needed
Documentation is completed in an accurate and timely manner on the appropriate forms
Engage in health promotion, prevention, and education activities (i.e., assess for risky behaviors and attempt behavior change).
Assess acute and chronic conditions that could result in rapid physiological deterioration of life-threatening instability.
Facilitate admissions, transfers, and/or discharges of patients
Manage the medication system, administer and document medication treatment.
Screen and monitor participants for medical problems/side effects.
Education & Experience
Must be able to be licensed as a LPN and able to work in the state of Nebraska
1 year of general nursing experience, preferably in IDD
A valid driver's license, good driving record, and reliable transportation is required.
Knowledge, Skills & Abilities
Must have excellent time management skills
Must possess well developed interpersonal and communication skills with the ability to communicate effectively and work collaboratively with all levels of staff within the organization.
Must be highly organized and responsive to immediate needs.
Must have sufficient knowledge of computer systems (Word, Excel and Outlook) to develop and maintain necessary programs to support quality assurance activities.

Good hearing and visual skills; ability to bend, stoop, sit, stand, reach, and lift up to 25 pounds. Other duties as assigned.
