Hospice Admission Nurse
Location: Kitsap County
Schedule: Full-Time |Monday - Friday
Compensation: $52.00 - $63.00 per hour + Benefits

We are seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments, primarily serving patients throughout Kitsap County. You will evaluate patient eligibility, review medical histories, coordinate with the care team, and provide education and emotional support to patients and families. Care is provided in private homes, adult family homes, assisted living communities, and skilled nursing facilities. This position will be based in Kitsap County, with occasional travel to Pierce, Thurston, and King counties as needed.

Primary Responsibilities
Conduct patient assessments addressing physical, emotional, and spiritual needs
Collaborate with physicians and the interdisciplinary team on care plans
Educate and support patients and families during hospice admission
Complete required documentation accurately and promptly
Perform hospice admissions and, when not scheduled, provide routine visits including death visits
Stay current with hospice best practices, policies, and regulatory standards
Requirements:
Active Washington State RN license and BLS certification
Valid driver’s license and reliable transportation
Minimum 1 year hospice or home health nursing experience preferred
Minimum 2–3 years healthcare-related experience required
Proficient with EMR systems and healthcare technologies
Strong judgment, initiative, and ability to work independently
Culturally sensitive and adaptable to diverse patient population
