BLS
4 years case management experience in an acute hospital setting

POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.
MINIMUM REQUIRED QUALIFICATIONS –
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8a-4:30p
START DATE – ASAP
Experience
RN License, BLS Certification, Managing patients as a nurse case manager, Acute care experience
