Position Summary

Collaborates with hospice, palliative care, and primary care medical teams, nursing staff, clinical staff, and facility staff to provide medical care to patients through direct visits, consultation, education, and research to ensure quality hospice care for hospice patients and families. Care will be delivered in compliance with all pertinent regulations including the hospice conditions of participation, corporate compliance and clinical licensure regulations while reflecting the national standards of care for hospice services.

Up to $10k Sign on Bonus available. This position will be full-time and based out of our Bismarck office location and serve the surrounding communities. Some travel is required as well as a rotation for On-call coverage.

Essential Functions

Provides disease and symptom management for patients

Determines continued patient eligibility through face-to-face visits

Consults with nursing staff regarding medical care of patients

Collaborates on education of patient care staff, referral sources, and the public

Qualifications

Licensed Nurse Practitioner OR Physician Assistant

Certifications, Licenses, Registrations Required

DEA (ND & MN)

Licensure in North Dakota and Minnesota

Current drivers license and auto insurance
