What You'll Do
As a Home Health Nurse, you will:
Provide skilled nursing care to patients in their homes.
Assess patient needs and develop individualized care plans.
Administer medications, treatments, and wound care as ordered.
Monitor patient conditions and communicate changes to physicians and the care team.
Educate patients and family members on disease management, medications, and healthy living.
Maintain accurate and timely clinical documentation.
Collaborate with therapists, physicians, caregivers, and other healthcare professionals to ensure quality outcomes.
What We're Looking For
Current RN license in good standing.
Valid driver's license and reliable transportation.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal abilities.
Ability to work independently while remaining connected to a supportive team.
Previous home health experience is a plus, but we welcome motivated nurses eager to learn.
