About This Position
The Field RN assists in planning, organizing and directing home health services. Initiates the home health plan of care and makes the necessary revisions as the patient status and needs change. Communicates changes to the Clinical manager, Case Manager, Physician and team members as needed.
Professional Duties
Responsible for the delivery of competent, patient centered care rendered in the patient’s residence.
Completes a comprehensive assessment of patients and families, including the OASIS when appropriate, to determine qualifications for home health services, skilled needs of patients, and services required to obtain quality outcomes for the patient.
In consultation with the Clinical Manager, develops a patient plan of care that outlines the care and services necessary to meet patient-specific needs identified in the comprehensive assessment.
Performs a comprehensive physical assessment and history of current and previous illnesses each visit as appropriate, including a complete medication review and progress towards the patient goals.
Provides patient-specific interventions and education based on the patient’s diagnosis and identified needs of the patient and representative.
Regularly communicates with the Clinical Manager and Case Manager with any updates, including progress towards goals and discharge.
Follows the plan of care and notifies the Clinical Manager, Case Manager, Physician, and the patient and representative when the plan of care is updated due to any significant change in the patient’s health status, including discharge. Participates in team meeting, quality improvement teams and in-services as required.
Performs other duties as assigned.
Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience
Be sensitive to factors that influence customers and co-worker’s situation including age, gender, culture, race and socioeconomic status. Be observant of others’ social cues (emotions) and respond appropriately
Summarize key information and provide written materials whenever possible
Qualifications
Current California RN License.
One year of prior professional nursing experience.
Basic computer skills.
Ability to manage time and stay organized. Excellent assessment and nursing skills.
Good communication skills, detail oriented, critical thinking and problem solving skills. Excellent customer service and telephone etiquette skills.
BLS required within 60 days of hire.
Current California Driving License and proof of insurance.
Physical Demands
Sitting: Approximately 25% of day
Standing: Approximately 50% of day
Walking: Approximately 25% of day
Lifting: 0 - 50 lbs (patients,equipment, supplies) - approximately 40% of day
Bending: Approximately 10% of day. Kneeling < 20%.
Hearing/Visual Acuity: Adequate for use with computers, telephone and or Blackberry -
Approx 50% of day
Computer: Highly technical work environment - Must be able to work ≥ 6 hours / day using keyboard, mouse and monitor
Reaching: Above head 75 degrees - approximately 10%.
Hand grip dexterity: Approximately 40% of day
