Overview
Supportive Care Specialist Group is seeking a compassionate and experienced Palliative Care Nurse Practitioner NP to provide patient-centered care in Seattle WA across hospital and LTAC settings.
This role offers the opportunity to practice with strong clinical autonomy while working within a collaborative interdisciplinary team. You will care for adult and geriatric patients with complex conditions and support them and their families through important care decisions.
Key Responsibilities
Conduct daily patient rounding in hospital and LTAC settings
Provide palliative care consultations and symptom management
Lead goals of care discussions with patients and families
Collaborate with physicians nurses and interdisciplinary teams
Develop patient centered care plans
Maintain timely and accurate clinical documentation
Qualifications
Required
Active Washington Nurse Practitioner NP license
Minimum 2 years inpatient hospital experience
Minimum 2 years primary care experience
Strong communication skills and compassionate bedside manner
Preferred
Experience in palliative care hospice or geriatrics
Bilingual Spanish speaking is a plus
