About This Position

DEPARTMENT: MEDICAL

JOB TITLE: LICENSED VOCATIONAL NURSE / SUPERVISOR

REPORT TO: DIRECTOR OF NURSING

BLOODBORNE PATHOGENS EXPOSURE: HIGH RISK

BASIC FUNCTION: Assists Clinicians by providing quality nursing care and accurate documentation. Provides supervision to medical assistants. Assists in scheduling follow-up of patients, oversee referral appointments and conducts CQI activities at the clinic. Performs other related duties as required.

REPRESENTATIVE DUTIES: The duties recorded below are intended to accurately represent the duties of the class and are not intended to cover every single duty of the job.

1. Assists clinicians in providing patient care

2. Obtains and records accurate and complete patient histories; ensures patient records are up-to-date; interviews patient to determine specific health complaint.

3. Ability to navigate and use an electronic health record.

4. Knowledge and implementation of community-based, patient-centered medical home (PCMH) process with appropriate coordination of care.

5. Performs various nursing tasks as permitted by license, including but not limited to giving injections, administering medications, collecting body fluids for testing, performing TB tests, performing basic health screening, performing EKGs, performing simple laboratory tests, etc.

6. Provides supervision to Medical Assistants to ensure that patient centered care is delivered effectively.

7. Monitors the effectiveness of services provided through the patient’s individualized plan of care.

8. Directs Medical Assistant staff in planning, implementing and evaluating the patient-centered care process.

9. Documents patient care in accordance with Innercare’ policies and procedures.

10. Attends daily team meetings/huddles, as member of the health care team.

11. Makes referrals to other agencies as appropriate.

12. Assists in developing and providing health education information and counseling to patients.

13. Maintains adequate and well-organized inventory of supplies and equipment.

14. Assists in gathering and maintaining statistical data.

15. Assists in scheduling appointments as necessary.

16. Attends staff meetings and participates in the planning and/or evaluation of patient care; provides recommendations as appropriate to enhance patient care.

17. Maintains required LVN license and CPR certification; attends and participates in educational opportunities in the clinic and outside in the community.

18. Operates a variety of office equipment including computer, typewriter, copier, calculator, fax machine, telephone, etc.; operates and maintains nursing/medical equipment and supplies.

19. Maintains a clean and pleasant environment for patients and staff.

20. Works with CQI Director for ensuring that all CQI activities are conducted in a timely manner.

21. Works with Clinic manager in meeting budget expectations for the clinic.

22. Performs other duties as may be assigned.

EDUCATION AND EXPERIENCE: Any combination equivalent to: graduation from high school and an approved vocational nursing program and experience closely related to the duties and responsibilities of the job. Must possess a current California Vocational Nurse license and CPR certification.

KNOWLEDGE AND ABILITIES: Knowledge of the methods, practices and terminology used in nursing care; knowledge of the use and care of medical/nursing supplies and instruments; knowledge of medical crisis protocol; knowledge of standard medical record-keeping procedures and regulations; knowledge of modern office practices and technology, including computers; knowledge of and skill in the proper use of English and Spanish languages; ability to learn, interpret and/or communicate rules, regulations, procedures, and policies; ability to follow oral and written directions; ability to provide effective leadership as assigned; ability to establish and maintain cooperative relationships with subordinates, co-workers, supervisors, professionals and patients; ability to learn new information and skills to enhance job performance; ability to prepare reports and records with accuracy and to maintain patient confidentiality as required; ability to make required calculations accurately; ability and willingness to work irregular hours, overtime, or weekends as necessary.

WORK DIRECTION, LEAD AND SUPERVISORY RESPONSIBILITIES: Assigned Medical assistant staff to supervise.

PHYSICAL EFFORT: Normal medical clinic environment. Work requires the ability to walk and stand for extended periods of time, and to exert moderate, though not constant, physical effort in the performance of nursing procedures, typically involving some combination of stooping, kneeling, crouching, and the lifting, carrying, pushing and/or pulling of objects and materials of moderate weight.

CONTACTS: Supervisor, co-workers, physicians, other Innercare personnel, pharmacists, patients, vendors, hospital personnel, Health Department personnel, and the general public.

WORKING CONDITIONS: Worker risks exposure to odors, toxic/poisonous agents, communicable disease, pathogenic substances. Work is performed under stressful conditions. Must comply with the Innercare Corporate Compliance Program.
