Lead clinical excellence within our rehab division as a Clinical Coordinator (RN/LPN), supporting high-quality resident care and strong team performance! In this role, you will partner closely with nursing leadership to coordinate care, guide clinical teams, and ensure best practices are consistently delivered. Position Summary

The Clinical Coordinator plays a key role in supporting clinical excellence across our Rehab Division. This position partners closely with therapy, nursing, and leadership teams to ensure coordinated, high-quality care delivery that aligns with our mission to celebrate life through exceptional service and personalized care. The Clinical Coordinator serves as a clinical resource, ensuring compliance, consistency, and a strong resident-centered approach in all aspects of care.

Key Responsibilities

Clinical Oversight Coordination

Collaborate with therapy and nursing teams to ensure coordinated care plans for residents receiving rehabilitation services

Serve as a clinical resource for staff regarding best practices, policies, and procedures

Assist in monitoring clinical outcomes and identifying opportunities for improvement

Support implementation and consistency of care standards across rehab services

Quality Compliance

Ensure compliance with all regulatory requirements, company policies, and clinical standards

Participate in audits, chart reviews, and documentation oversight to ensure accuracy and timeliness

Assist in tracking and addressing clinical quality indicators and risk management concerns

Team Collaboration Communication

Partner with Director of Nursing, Rehab Director, and interdisciplinary teams to support resident outcomes

Communicate effectively with staff to ensure clarity in care plans and expectations

Support coordination of care transitions and discharge planning as needed

Staff Support Development

Provide guidance, coaching, and support to nursing staff

Assist in onboarding and ongoing education related to clinical practices and expectations

Promote a positive, collaborative team environment aligned with our culture

Operational Support

Assist with staffing coordination and clinical coverage as needed

Support leadership with clinical initiatives, process improvements, and special projects

Participate in meetings and provide insights on clinical operations and opportunities

Qualifications

Active RN or LPN license in the state of Nebraska

Minimum of 2 years of clinical experience (post-acute, rehab, or long-term care preferred)

Strong understanding of clinical processes, documentation, and regulatory requirements

Excellent communication and interpersonal skills

Ability to work collaboratively across interdisciplinary teams

Strong organizational skills with attention to detail
