We are currently seeking an experienced, compassionate, and motivated Full-Time Field Registered Nurse (RN) to join our growing clinical team in Lubbock, Texas.

Position Summary
The Field Registered Nurse (RN) will primarily provide Hospice and Home Health nursing services to patients throughout our service area, covering approximately a 60-mile radius around Lubbock.
The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering outstanding patient-centered care.
Hospice experience is required.

Essential Responsibilities Clinical Care
Provide skilled nursing services to Hospice and Home Health patients in accordance with physician orders and agency policies.
Perform comprehensive patient assessments and reassessments as required.
Develop, implement, and evaluate individualized patient care plans.
Coordinate care with physicians, families, caregivers, and interdisciplinary team members.
Monitor patient conditions and promptly communicate significant changes to the appropriate healthcare providers.
Provide education and support to patients and family members.
Documentation & Compliance
Complete all required clinical documentation accurately and timely within the Electronic Medical Record (EMR) system.
Ensure compliance with Medicare, State, Federal, and agency regulations.
Participate in quality improvement and utilization review activities.
Maintain professional standards and agency accreditation requirements.
Leadership & Team Collaboration
Collaborate with clinical staff and management to ensure quality patient outcomes.
Participate in clinical case conference and IDG meetings.
Assist in maintaining effective communication among team members and referral sources.
Support continuous quality improvement initiatives.
Attend educational programs, conferences, and professional development activities.
Qualifications Required
Current and unrestricted Registered Nurse (RN) license in the State of Texas.
Minimum one (1) year of Home Health and Hospice experience.
Strong assessment and clinical decision-making skills.
Excellent verbal and written communication skills.
Valid driver's license, reliable transportation, and current automobile insurance.
Physical Requirements
Ability to travel regularly throughout the service area.
Ability to perform patient assessments in home environments.
Ability to lift up to 30 pounds and assist with patient transfers up to 50 pounds when necessary.
Ability to stand, walk, bend, stoop, reach, and sit for extended periods.
Sufficient visual, auditory, and physical capabilities to safely perform nursing duties.
