Requirements
- Active LPN/LVN license
- Experience in long-term care (LTC) settings preferred/required as applicable to the assignment
- Ability to provide safe, compassionate nursing care to patients in accordance with facility policies and scope of practice
Responsibilities
- Provide direct patient care within the LPN/LVN scope
- Monitor patients, report changes in condition, and follow care plans
- Assist with activities of daily living and support resident comfort and wellbeing
- Document care and communicate effectively with the care team
- Maintain safe medication practices and assist with medication administration as required
