Job Description:
We are seeking a skilled and compassionate Home Health LVN to join our team. The Home Health LVN will be responsible for providing nursing care to patients in their homes, under the direction of a registered nurse or physician. The ideal candidate will have experience in home health care, excellent communication skills, and a passion for helping others.
Responsibilities:
Assess patient's health status and develop care plans
Administer medications and treatments as prescribed by a physician
Monitor patients' vital signs and report any changes to the RN or physician
Provide wound care and other nursing interventions as needed
Document patient care and progress in electronic medical records
Communicate with patients, families, and other healthcare providers as needed
Participate in interdisciplinary team meetings and case conferences
Adhere to all state and federal regulations and agency policies and procedures
Qualification & Requirements
Current LVN license in the state of Texas
Minimum of one year of experience in home health care
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Strong critical thinking and problem-solving skills
Valid driver's license and reliable transportation
Ability to pass a background check and drug screening
Wound care 2-3 x week
