About the Role:
As an LPN on our team, you’ll play a key role in delivering hands-on care and supporting residents’ overall well-being. You’ll collaborate closely with nurses, physicians, and support staff to ensure high-quality care in a structured and positive environment.
What You’ll Do as a Licensed Practical Nurse (LPN):
Administer medications and treatments per physician orders
Track and document resident conditions and changes
Support care planning and follow individualized care plans
Provide direct nursing care, including wound and catheter care
Communicate effectively with interdisciplinary teams
Educate residents and families on care and treatment plans
Respond promptly and effectively to clinical needs or emergencies
Assist in guiding and supporting CNAs
What We’re Looking For in Licensed Practical Nurses (LPN):
Active LPN license or Compact LPN license
Strong attention to detail and documentation
Team-oriented mindset with the ability to work independently
Compassionate approach to resident care
Clear communication and professionalism
