Position Summary
The Registered Nurse (RN) provides skilled nursing care to patients in their homes by assessing patient needs, developing individualized care plans, coordinating services, and delivering compassionate, evidence-based care. This role requires strong clinical judgment, excellent communication, and the ability to work independently while collaborating with physicians and the interdisciplinary team.
Responsibilities
Complete comprehensive Start of Care (SOC), ROC, Recertification, Follow-up, and Discharge assessments.
Develop, implement, and revise individualized Plans of Care based on patient needs.
Perform skilled nursing assessments, treatments, medication management, and patient education.
Regularly evaluate patient progress and update care plans as needed.
Coordinate patient care with physicians, therapists, home health aides, and other healthcare professionals.
Communicate changes in patient condition and obtain physician orders when necessary.
Complete accurate and timely clinical documentation in WellSky.
Provide health education and counseling to patients and caregivers.
Supervise Home Health Aides and complete required supervisory visits.
Participate in case conferences, quality improvement initiatives, and on-call rotation as assigned.
Ensure patients receive appropriate equipment and services to support their care.
Qualifications
Required
Current California Registered Nurse (RN) license
Graduate of an accredited School of Nursing
Current CPR/BLS Certification
Valid California Driver's License
Reliable transportation with current auto insurance
Ability to travel to patient homes within assigned territory
Strong assessment, clinical, communication, and documentation skills
Ability to work independently with minimal supervision
Preferred
Minimum 2 years of nursing experience
Home Health, Public Health, Hospice, or Community Health experience
Case Management experience
Experience using WellSky or other Home Health EMR systems
Bilingual is a plus
