Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home health care team. As a Home Health Nurse, you will provide expert nursing care directly in patients' homes, promoting health, independence, and well-being. Your role is vital in delivering personalized, high-quality care that meets each patient’s unique needs. Duties

Conduct comprehensive patient assessments including vital signs, physical examinations, and medication reconciliation to develop tailored care plans.

Administer medications via various routes such as injections, IV infusions, and catheter care while adhering to strict safety protocols.

Manage complex wound care, including dressing changes and infection control measures for patients with chronic wounds or post-surgical needs.

Provide specialized care for patients with Alzheimer’s disease, dementia, or memory impairments, ensuring safety and comfort during visits.

Assist with activities of daily living (ADLs), including tube feeding, mobility assistance with Hoyer lifts, and respiratory support such as ventilator management.

Document all patient interactions accurately using electronic health record (EHR) systems like Epic or eClinicalWorks while maintaining compliance with HIPAA regulations.

Collaborate with interdisciplinary teams on case management plans for patients requiring acute pain management, hospice care, or long-term care services.

Educate patients and caregivers on health coaching topics such as medication administration, nutrition, infection control, and chronic disease management.

Coordinate with physicians and other healthcare providers for diagnostic evaluations, laboratory specimen collection/processing, and follow-up care.

Ensure compliance with all long-term care regulations and safety standards across diverse settings including skilled nursing facilities and assisted living communities.

Experience

Valid licensure as a Licensed Practical Nurse (LPN) in the state of practice.

Prior experience working in home health environments or community-based settings is highly preferred.

Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation.

Experience providing specialized care including IV infusion therapy, sterile processing techniques, and catheterization procedures.

Background in caring for geriatric populations with complex needs such as dementia care or memory impairment cases.

Knowledge of medical terminology related to internal medicine, geriatrics, pediatrics (including toddler and infant care), and emergency medicine scenarios like Level I Trauma Center experience.

Familiarity with ICD coding (ICD-9/10), CPT coding for procedures, and documentation standards aligned with NCQA standards is advantageous.

Ability to work independently while maintaining excellent caregiver communication skills to ensure comprehensive patient support across diverse healthcare environments.

Pay: $29.00 - $30.00 per hour
