We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) - Field Nurse FOR HOME HEALTH VISITS to join our dynamic healthcare team. Responsibilities
Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation.
Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
Conduct specialized procedures such as catheterization, tube feeding management, venipuncture, and sterile dressing changes.
Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs.
Provide education on disease management, medication administration, and health coaching for patients and their families.
Manage urgent situations including acute pain episodes, airway management, and triage assessments in various settings such as assisted living, home care, or outpatient clinics.
Requirements
Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
Proven experience in critical care environments such as ICU, emergency room, or trauma centers is highly preferred.
Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth.
Experience working with individuals with developmental disabilities, dementia care (including Alzheimer's), memory care, and behavioral health is desirable.
Knowledge of ICD-9/ICD-10 coding for accurate documentation and billing purposes.
Ability to perform complex patient assessments and diagnostic evaluations confidently.
Strong understanding of infection control practices, sterile techniques, and aseptic procedures.
Excellent communication skills to effectively educate patients and coordinate with healthcare teams.
Flexibility to travel within assigned regions for home visits or community-based care.
