The Role
We are seeking a quick-thinking, responsible LPN to join our nursing team. In this role, you will provide high-quality skilled nursing care while fostering a warm, homelike atmosphere. You will work closely with residents and their families to ensure their physical, social, and emotional needs are met with dignity.
Key Responsibilities
Clinical Excellence: Assess resident health needs, administer medications, and perform skilled treatments (wound care, catheter care, etc.) in accordance with state guidelines.
Person-Centered Planning: Collaborate with a multidisciplinary team to develop individualized care plans that attain the highest practicable physical and psycho-social well-being for each resident.
Communication & Leadership: Serve as a bridge between residents, families, and physicians. Provide clear status reports and ensure interventions are implemented with care.
Empowerment: Educate residents and families about medical conditions while advocating for the resident’s personal preferences.
Quality Advocacy: Ensure compliance with all state and federal regulations while maintaining the "Grandview Standard" of compassion and integrity.
Qualifications
Education: Graduate of an accredited nursing program.
Licensure: Valid LPN license in the State of Wisconsin (registered on the WI registry).
Skills: Strong clinical knowledge, organizational skills, and the ability to stay calm under pressure.
Mindset: A patient-centered approach with excellent interpersonal skills. We value nurses who are empathetic, collaborative, and dedicated to geriatrics.
CPR Certification: Preferred.
